- 6 champs texte
- 1 champ email
- 2 menus déroulants
- 1 boite texte
- Fonction upload de fichier (txt, doc, pdf, odt,...)

Prestation souhaitée :
- Création formulaire HTML
- Création du script upload avec renvoi par email

Pas d'intégration à un site internet, livraison des fichiers seuls.

Choix du prestataire en fonction du prix le plus bas.
